Disclosure Document 2013-2014

I am thrilled to have your child in 4th
 grade this school year! We will have many memorable
experiences while learning. Please read this document carefully so that you will be ready
for the school year.

Grade Level Description, Goals, and Objectives
We are using the Common Core, which outlines standards that are essential for all students
at each elementary grade level. For a detailed list of objectives, see your curriculum
brochure or Utah State Office of Education’s website at:

Instructional Materials
 Reading/Language Arts: Harcourt StoryTown
 Math: Houghton Mifflin Math Expressions Common Core

Instructional Practices
Typical instructional practices will include lecture, discussions, and, among others, group and
independent learning activities such as hands-on projects and presentations.

Grade Level (Course) Requirements
To meet grade level standards, students are expected to demonstrate proficiency in what
has been taught with minimal teacher assistance. Punctuality and daily attendance are
critical toward meeting these goals. Please plan vacations accordingly.

Homework/Grading Procedures
Homework will be assigned as reinforcement and/or remediation of class material.
Additionally, homework may be used as a prescriptive measure for those students needing
further practice. Advanced students will be given opportunities to do assignments or
projects outside of class. Each student has a homework folder and an agenda (planner) that
will come home each night.
Homework is as follows:
 Reading – 30 minutes each night, including weekends.
 Math – Most nights one math sheet will be sent home for students to practice the
skill taught in class that day.
 Spelling – It is expected that students practice spelling each night.
 Independent Projects – I require two book reports during the year. More
information will be sent home regarding these projects.  Agenda/Planner – The agenda is used as a tool of communication to parents and as a
reminder of homework for students. It is expected that parents initial or sign
students’ agendas each night.

Grading
 Each student’s grade will be computed using a weighted point system. Generally,
there are a few chances to earn some extra credit points during a grading quarter.
 The weighting scale is as follows:
• Classwork 10% • Participation/readiness 10% •
Quizzes 10% • Homework 15% • Projects 25% • Tests 30%
 Some late assignments MAY be accepted up to a week after the due date with
maximum allowable credit of 50%. Students with an excused absence will be given
extra time, depending on the length of the absence.
